Health Requirements for Athletic Participation
All students in grades 7-12 who desire to participate in interscholastic athletics must be cleared through the school Nurse’s Office. No student will be allowed to try out, practice or participate with any team without a current physical examination on file and if they have not completed registration through FamilyID. There are no exceptions.
Sports Physicals Offered at School
A current physical is one that has been completed within one year of the month in which the sport starts. The district offers sports physicals with the Board-appointed school physician primarily during the summer. These physicals are free of charge. Prospective student-athletes are encouraged to complete their physical during this time, as this will provide eligibility for the entire year.
This means that if you are a fall, winter or spring athlete you should complete a sports physical on one of the dates provided. Additionally, if you are a middle school student seeking the APP (athletic placement process) in order to play at the high school level, these physicals will meet that requirement also. These physicals will take place in the nurse’s office at the high school.
Student-athletes who do not complete a physical with the school physician will need to obtain one from their family doctor. In this event, the Physical Form must be completed and returned directly to the school nurse.
Registration & Health Requirements
Register via FamilyID – The Athletics Department uses the FamilyID online platform for registration and for students to provide health documentation. FamilyID replaced the Gray Card that was used in the past. When you register through the secure FamilyID system, it keeps track of your information in your FamilyID profile. You only need to enter your information only once for each family member for multiple uses and multiple programs. Learn more about FamilyID and creating a profile
Physical Form – A current physical must be on file with the nurse’s office. A current physical is one that has been completed within 12 months of the month that the sport starts. Physicals are submitted through FamilyID.
*Important note: During the summer, paperwork for all student-athletes in grades 7-12 should be submitted to the Niskayuna HS Nurse’s Office, either in-person or via U.S. Mail (1626 Balltown Road, Niskayuna, NY 12309). During the school year, paperwork for middle school students should be submitted to the nurse at their respective school.
