Procedure for exiting students or change of address

Change of address out of district

If you know that your family is planning to move outside of the Niskayuna Central School District, please complete a Student Exit Form and return it to the main office of the school that your child attends before the end of the school year. Click here to access the form. Print forms are also available in the main office of each school.

Please note, students leaving the district must return the following items to their school’s main office:

  • Chromebook
  • Library materials
  • Textbooks
  • Any other district owned materials

Change of address within district

If your family will be relocating within the Niskayuna Central School District, please notify the District Registrar, Dana Mazzone (dmazzone@niskyschools.org) to let her know of your move.  In order for her to update your address in PowerSchool, you will need to send her one item from group A and one item from group B from the list below:

Proof of Residency Documentation
(Must submit a minimum of 2, one from group A and one from group B)

Group A

  • Lease OR Mortgage Statement
  • Homeowner’s Insurance Bill
  • Closing Statement
  • Contract of Sale
  • Property Tax Bill/Receipt
  • Other (Must be approved by Registrar before submittal)

Group B

  • Utility or other bills
  • Income Tax
  • Membership documents based on residency
  • Voter Registration documents
  • Local, State or Government issued ID documents
  • Pay Stub
  • Official driver’s license, learner’s permit or non-driver identification
  • Sworn Statement by third-party (landlord, etc.)
  • Evidence of Custody of Child (custody orders, guardianship papers, etc.)
  • Other (Must be approved by Registrar before submittal)
